The settlement resolves Leo Woytach, et al. v. Drug and Alcohol Treatment Services, Inc., Case No. 2025-CV-03681, over a data breach affecting protected health information between October 5, 2024 and October 6, 2024. Class members may elect either up to $5,000 for documented out-of-pocket expenses or an alternate flat cash payment that adjusts based on total valid claims, and may also enroll in one year of medical identity-theft monitoring through CyEx. The defendant disagrees with the plaintiff's claims and denies any wrongdoing.
